    • Is and other options.The default option is ‘is,’ and we will keep it as is.
    • Select Options.In this final field, select the ‘Technical Support Inquiry’ option.

    To include your own questions, click on each field and modify the text using the customization panel on the left.

    In this tutorial, we will create a quiz focused on health and wellness as our example.

    We are updating the first question from ‘Which one of these is not like the others?’ to ‘What is your primary fitness goal?’

    Next, check the ‘Required’ box to ensure that users must answer this question.

    You can also change the style of the radio buttons. We will keep it as ‘Simple’, but feel free to use images or install the ‘Button’ add-on for additional button styles.

    Now, scroll down the customization panel to modify the options for this question.

    We will change the options to ‘Weight loss,’ ‘Muscle gain,’ and ‘General health.’ The default template includes four options, so we need to adjust that as well.

    To delete an option, simply click the ‘-’ button next to the option field.

    After clicking the ‘-‘ button, you’ll notice in the live preview that Formidable Forms has successfully removed the selected option.

    Next, create related questions for each option, as we will be setting up conditional logic based on the user’s choice among the three options.

    You can repeat this process to add additional questions. For example, here’s how our extra question for the ‘Weight loss’ option appears:

    For the ‘Muscle gain’ option, consider asking about the user’s dietary preferences.

    Given the variety of dietary preferences available, you can include some popular options and an ‘Other’ field for users to specify their preferences.

    Simply click ‘Add other’ at the end of the options section to include this feature.

    Once you do this, an ‘Other’ radio button along with a text box will appear. If you haven’t done so yet, feel free to click the ‘-‘ button for the ‘D’ option.

    Here’s an example of how that might look:

    If there are extra fields that are unnecessary, you can easily delete them.

    To remove a field, first click on it, then click the three-dot button located in the top right corner.

    When the menu opens, simply select ‘Delete’ to remove the field.

    Now, let’s proceed to set up the conditional logic.

    First, navigate to the second question regarding how many times users plan to exercise each week. Click on the field to proceed.

    Next, scroll down the customization panel and expand the ‘Conditional Logic’ section.

    In the ‘Conditional Logic’ area, you can set up various drop-down menus.

    You will need to display the ‘How many days a week will you exercise?’ field when users select the ‘Weight loss’ option. Here are the necessary settings:

    • Show or hide this field.This is where you can choose to show or hide the ‘How many days a week will you exercise?’ field. You can set it to ‘Show this field.’
    • Any or all.In this dropdown, you can specify whether any or all of the conditions must be met. Let’s keep it set to ‘any.’
    • Select field.Here, you will select one of the other fields in the form to determine the subject of the condition. Let’s choose ‘What is your primary goal?’
    • Equals and other options.By default, the setting is ‘equals.’ You can leave it as it is.
    • Select option.In this final field, you should select ‘Weight loss,’ as this question should be shown to users who choose that option.

    If you only need to set one condition, you can either click the ‘-‘ button or leave it unchanged.

    Here’s an example of what the final configuration may look like:

    Next, let’s configure conditional logic for the question ‘Do you prefer home or gym exercise?’. In this guide, we will display this question if users select any option other than the ‘How many days a week will you exercise?’ field.

    Click on the ‘Do you prefer home or gym exercise?’ field to access the settings for conditional logic.

    In the conditional logic settings, you will want to establish two conditions:

    • ‘How many days a week will you exercise?’ ‘equals’ ‘1-2 times a week’
    • ‘How many days a week will you exercise?’ ‘equals’ ‘3-4 times a week’

    These settings will keep the ‘Do you prefer home or gym exercise?’ field hidden for users who choose the ‘Once a week’ option. For those users, we can directly recommend home exercises.

    You can apply this process to all your other fields. Once completed, click the ‘Update’ button.

    You can now embed the quiz form into your WordPress site by clicking the ‘Embed’ button at the top of the form builder.

    A popup will appear with various embedding options.

    You can choose to use an existing page, create a new one, or manually add the form using a shortcode.
